«Эталонные» результаты («textbook outcomes») в колоректальной хирургии (систематический обзор)

ЦЕЛЬ: проанализировать существующие определения «textbook outcome» (ТО) в хирургии колоректального рака и обосновать необходимость их унификации.

ПАЦИЕНТЫ И МЕТОДЫ: проведен систематический обзор в соответствии с рекомендациями PRISMA. Поиск проведен в базах PubMed, Cochrane Library и eLIBRARY за период 2010–2025 гг. Включены оригинальные исследования, оценивавшие ТО у взрослых пациентов после хирургического лечения колоректального рака. Качество исследований оценивали по шкале Newcastle-Ottawa.

РЕЗУЛЬТАТЫ: включено 16 исследований (335 575 пациентов). Частота достижения ТО варьировала от 28,5% до 77,4%. Выявлена выраженная гетерогенность определений ТО. Наиболее частыми компонентами являлись R0-резекция, лимфодиссекция ≥ 12, отсутствие тяжелых осложнений (Clavien-Dindo ≥ III), повторных госпитализаций и летальности (30/90 дней), а также длительность госпитализации (фиксированные значения либо 75-й перцентиль). Достижение ТО ассоциировалось с улучшением выживаемости (снижение риска смерти на 40–55%). Значимыми факторами были хирургический доступ, ERAS, возраст, коморбидность, стадия, неоадъювантная терапия, тип страховки и объем операций.

ЗАКЛЮЧЕНИЕ: ТО — валидный инструмент оценки качества хирургического лечения колоректального рака, связанный с долгосрочной выживаемостью. Отсутствие унифицированного определения ограничивает сопоставимость результатов. Предложенное стандартизированное определение (R0-резекция, лимфодиссекция ≥ 12, отсутствие тяжелых осложнений, 30-дневной летальности и повторных госпитализаций, длительность пребывания в пределах 75-го перцентиля) может служить основой для международного консенсуса.
