Определение порогового значения коэффициента жесткости при эндоректальной компрессионной ультразвуковой эластографии для аденокарцином прямой кишки. ПАЦИЕНТЫ И МЕТОДЫ. В исследование включено 45 больных [средний возраст ± s 61 ± 9,5 (38-80 лет), 29 женщин и 16 мужчин] с подтверждёнными при биопсии новообразованиями прямой кишки. Эндоректальную ультрасонографию проводили на приборе Hitachi Hi Vision Preirus (Япония). Все больные оперированы в объёме трансанальной эндомикрохирургии. РЕЗУЛЬТАТЫ. При гистологическом исследовании операционных препаратов в 31 выявлены аденомы (в 5 препаратах - интраэпителиальный рак), в 14 - инвазивная аденокарцинома.. Точка отсечения коэффициента жесткости составила 5,7. При этом чувствительность ультразвуковой компрессионной эластографии составила 0,94 (95 %CI 0,68-0,99), специфичность - 0,92 (95 %CI 0,74-0,98), положительная прогностическая значимость - 0,90 (95° % CI 0,84-0,96), отрицательная прогностическая значимость - 0,96 (95 % CI 0,85-0,99). ЗАКЛЮЧЕНИЕ. Полученные результаты свидетельствуют о высокой диагностической ценности компрессионной эластографии, что делает ее перспективным методом выявления скрытой малигнизации в крупных аденомах прямой кишки.</p></abstract><trans-abstract xml:lang="en"><p>AIM. To establish cut-off point of strain ratio for rectal adenocarciomas. PATIENTS AND METHODS. Forty-five patients (29 female) at mean age ± SD of 61 ± 9.5 (range, 38-80) years with histologically confirmed rectal neoplasia were included into study. Endorectal Ultrasonography accomplished by strain elastography was performed by Hitachi Hi Vision Preirus (Japan). All patients were treated by Transanal Endoscopic Microsurgery. RESULTS. Pathologic examination of operative specimen revealed 31 adenomas (5 intraepithelial carcinomas) and 14 invasive carcinomas. Cut-off point of Strain Ratio was 5,7 with sensitivity of 0.94 (95 °%CI 0,68-0,99), specificity of 0.92 (95 %oCI 0,74-0,98), positive predictive value of 0,90 (95 % CI 0,84-0,96) and negative predictive value of 0,96 (95 %CI 0,85-0,99). CONCLUSION.
