По данным литературы, до 15 % взрослого населения является носителями токсигенных штаммов клостридий. Данные о носительстве подобных микроорганизмов у медицинских работников и в частности, работников колопроктологических стационаров в России отсутствуют. ЦЕЛЬ. Оценить носительство токсин - продуцирующих штаммов клостридий среди медицинских работников в колопроктологи ческом стационаре. МАТЕРИАЛЫ И МЕТОДЫ. В анализ включено 39 медицинских работников отделения онкологии и хирургии ободочной кишки ФГБУ «Государственного научного центра колопроктологии им. А.Н. Рыжих» Минздрава России. Материал был исследован на наличие глутаматдегидрогеназы токсинов A и B C.difficile с помощью иммунохроматографического метода. Также проводилось выделение культуры возбудителя в бескислородной среде. Для детекции микроорганизмов использовали методику матрично-ассоциированной лазерной десорбционной ионизационной времяпролетной масс - спектрометрии. РЕЗУЛЬТАТЫ. Всего было исследовано 39 образцов просветных фекалий. Позитивные тесты на токсины А и В C.difficile были в 28 (71,8 %) пробах. В 24 (61,5 %) из 39 образцов просветных фекалий удалось выделить культуру представителей рода клостридий. Из них 17 (70,8 %) проб имели позитивные тесты на токсин C.difficile и 7 (29,2%) - негативные. Среди детектированных микроорганизмов были обнаружены не только C.difficile (3), но и другие представители этого рода: C.perfringens (17), C.bifermentas (4), C.tertium (3), C.disporicum (2), C.sordellii (2). ВЫВОДЫ. C.difficile идентифицирована в 3 (7,7 %) случаях, Носительство представителей рода клостридий у медицинского персонала оказалось 71,8 %, что почти в 5 раз выше, чем в среднем в популяции. Профессиональная деятельность сопряжена с более высоким уровнем контаминации токсин - продуцирующих бактерий рода клостридий.</p></abstract><trans-abstract xml:lang="en"><p>INTRODUCTION. The spread of microorganisms of the genus Clostridium in the population is 15 %. This phenomenon has not been studied in coloproctology department in Russia. AIM. To estimate the spread of toxin - producing strains of Clostridium spp. among the medical staff in coloproctology department. MATERIALS AND METHODS. There were analysed 39 of intraluminal faeces. The material was examined for the presence of glutamate dehydrogenase, toxins A and B of C.difficile using the immunochromatographic method. To detect microorganisms a matrix-activated laser desorption ionization time-of-flight mass spectrometry technique was used. RESULTS. Positive tests for toxins A and B of C.difficile were in 28 (71,8 %) of 39 samples. We have identified the culture of the genus Clostridium in 24 (61,5 %) of the 39 samples of intraluminal faeces. 17 (70,8 %) of these samples had positive tests for C.difficile toxins and 7 (29,2 %) was negative. In addition to C. difficile (3) others Clostridium were identified: C.perfringens (17), C.bifermentas (4), C.tertium (3), C.disporicum (2), C.sordellii (2). CONCLUSION. C.difficile was identified in 3 (7,7 %) cases. The spread of Clostridium microorganisms in medical stuff is almost 5 times higher than the average in the population.
