ВВЕДЕНИЕ. Желудочно-толстокишечный свищ - общепринятое определение патологического сообщения между желудком и толстой кишкой. Данная патология относится к редким осложнениям, может носить как первичный (спонтанный), так и вторичный (ятрогенный) характер и его можно заподозрить при наличии комплекса типичных жалоб пациента. Чаще всего выявить его позволяет ирригоскопия, но большое значение имеют и такие методы, как компьютерная томография и эндоскопические методы диагностики (гастроскопия, колоноскопия) с проведением биопсии. КЛИНИЧЕСКИЙ СЛУЧАЙ. Представлен клинический случай диагностики первичного желудочно-толстокишечного свища у мужчины 64 лет с аденокарциномой левого изгиба ободочной кишки. Жалобы пациента и клиническое обследование не выявили характерных признаков данного осложнения. При проведении колоноскопии выявлено патологическое сообщение между толстой кишкой и желудком, которое подтверждено при гастроскопии и компьютерной томографии. По результатам биопсии верифицирована аденокарцинома толстой кишки Пациенту выполнено радикальное хирургическое вмешательство, морфологически подтверждена аденокарцинома толстой кишки с формированием свища в желудок. ЗАКЛЮЧЕНИЕ. Морфологическое подтверждение расположения первичной опухоли в толстой кишке с ее прорастанием в соседний орган - желудок и формированием патологического сообщения - свища позволяет диагностировать первичный толстокишечножелудочный свищ. Такое определение делает понятным как механизм формирования свища, так и указывает на локализацию первичной опухоли.

INTRODUCTION. The gastro-colonic fistula: a common definition of a pathological communication between the stomach and colon. This pathology is a rare complication. Fistula can be primary (spontaneous) or secondary (iatrogenic) and can be suspected by the presence of typical symptoms. Most often it allows by the barium enema. Computer tomography and endoscopy (colonoscopy, gastroscopy) in combination with biopsy also have a certain diagnostic value. Clinical case: We present a case report of gastro-colonic fistula in a 64-year-old men patient with colon adenocarcinoma. Symptoms and clinical examination did not reveal typical signs of this complication. Colonoscopy revealed abnormal communication between the colon and stomach. It was confirmed by gastroscopy and computed tomography. Biopsy verified colon adenocarcinoma. The patient underwent radical surgery. Morphological study confirmed colon adenocarcinoma with fistula formation into the stomach. CONCLUSION. Morphological confirmation of the neoplasm grows from the originating organ (colon) to the other (stomach) with the formation of the pathologicalfistulous tract allows the diagnosis of primary colono-gastric fistula. This definition makes clear the mechanism of the fistula's formation and indicates the localization of the primary tumor.
