ЦЕЛЬ: сравнить результаты открытой геморроидэктомии и степлерной геморроидопексии у пациентов с геморроем III и IV стадии.

ПАЦИЕНТЫ И МЕТОДЫ

ПАЦИЕНТЫ И МЕТОДЫ: данное проспективное сравнительное наблюдательное исследование проводилось с марта 2023 года по март 2025 года. Сравнивались 26 пациентов, перенесших открытую геморроидэктомию, и 26 пациентов, перенесших степлерную геморроидопексию. Оцениваемые параметры включали время операции, послеоперационную боль (ВАШ на 1-м и 3-м днях после операции), потребность в анальгетиках, сроки восстановления дефекации, задержку мочи, послеоперационное кровотечение, продолжительность пребывания в стационаре, возвращение к работе и результаты последующего наблюдения.

РЕЗУЛЬТАТЫ

РЕЗУЛЬТАТЫ: геморрой III стадии составил 88,5% случаев. Время операции было одинаковым в обеих группах (медиана: 60 минут). Послеоперационная боль и потребность в анальгетиках достоверно не различались. Частота задержки мочеиспускания была значительно выше в группе с использованием степлера (34,6%) по сравнению с группой с открытым доступом (11,5%). Другие параметры, включая сроки восстановления трудоспособности и осложнений, были сопоставимы.

ЗАКЛЮЧЕНИЕ

ЗАКЛЮЧЕНИЕ: как открытая геморроидэктомия, так и степлерная геморроидопексия эффективны в лечении геморроя поздних стадий. Однако степлерная геморроидопексия продемонстрировала более высокую частоту задержки мочи. Хирургический подход может быть индивидуализирован с учетом профиля пациента и доступности ресурсов.

AIM

AIM: to compare the outcomes of open haemorrhoidectomy and stapled hemorrhoidopexy in patients with Grade III and Grade IV haemorrhoids.

PATIENTS AND METHODS

PATIENTS AND METHODS: this prospective observational comparative study was conducted at Kasturba medical college, Manipal, from March 2023 to March 2025. 26 patients undergoing open haemorrhoidectomy and 26 patients undergoing stapled hemorrhoidopexy were compared. Parameters assessed included operative time, postoperative pain (VAS on POD 1 and 3), analgesic requirement, bowel activity, urinary retention, postoperative bleeding, duration of hospital stay, return to work, and follow-up outcomes.

RESULTS

RESULTS: grade III haemorrhoids accounted for 88.5% of cases. Operative time was similar in both groups (median: 60 minutes). Postoperative pain scores and analgesic needs showed no significant difference. Urinary retention was significantly higher in the stapled group (34.6%) compared to the open group (11.5%). Other parameters, including return to work and complication rates, were comparable.

CONCLUSION

CONCLUSION: both open haemorrhoidectomy and stapled hemorrhoidopexy are effective in treating advanced haemorrhoids. However, stapled hemorrhoidopexy demonstrated a higher incidence of urinary retention. Surgical approach can be individualized based on patient profile and resource availability.
