Внекишечные проявления (ВКП) с поражением суставов и кожи изначально отмечены у 7/46 (3,2%) больных. Показанием к назначению УПА были: резистентность к терапии глюкокортикоидами (ГК), неэффективность ранее проводимой базисной терапии и терапии генно-инженерными биологическими препаратами (ГИБП). Оценку эффективности проводили с помощью индекса Мейо, эндоскопического индекса Schroeder, а также динамики изменения показателей гемоглобина, СРБ и СОЭ. УПА назначали в соответствии с инструкцией по применению: 45 мг для индукции и 15/30 мг/день для поддерживающей терапии. Критериями оценки служили: частота наступления раннего первичного клинического ответа в течение 1 недели, частота достижения клинического ответа/ремиссии и эндоскопического ответа/ремиссии, динамика ВКП и частота нежелательных явлений (НЯ) после индукционного курса длительностью 8 недель и поддерживающей терапии через 48 недель.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ: все 46 пациентов, включенных в исследование, завершили индукционный курс УПА в течение 8 недель. Ранний клинический ответ в течение 1 недели с сокращением частоты стула до 3 раз и менее, исчезновением крови в стуле был зарегистрирован у 25 (54,3%) пациентов. После окончания индукционного курса у 34 (73,9%) пациентов отмечена клиническая ремиссия, у 3 (6,5%) пациентов достигнут клинический ответ, в 9 (19,6%) случаях клиническая активность сохранялась. Нормализация СРБ и СОЭ и эндоскопическая ремиссия выявлены у 24 (52,2%) пациентов, в 9 случаях констатирована первичная неэффективность с сохранением умеренной и выраженной активности воспалительного процесса. К моменту окончания индукционного курса отмечено уменьшение выраженности ВКП у 4/7 (57,1%) пациентов. Из НЯ зарегистрирован 1/46 (0,46%) случай герпес zoster, что послужило причиной отмены препарата в рамках индукционного курса. Результаты поддерживающей терапии УПА на 48 неделе были оценены у 16 пациентов. Клиническая ремиссия в течение года терапии сохранялась в 14/16 (87,5%), при этом 11/16 (68,7%) из них получали УПА в поддерживающей дозировке УПА 30 мг в день, а 3/16 (31,3%) пациента — 15 мг в день. В 2/16 (12,5%) случаях отмечено сохранение клинической активности. У 10/16 (62,5%) пациентов отмечена эндоскопическая ремиссия, что выражалось в снижении эндоскопического индекса Schroeder до 1 и менее баллов, при этом 8 пациентов получали поддерживающую дозу препарата 30 мг в день, 2 пациента — 15 мг в день. У остальных пациентов в 4/16 (25%) (3 — УПА 30 мг, 1 — УПА 15 мг) случаях отмечена минимальная активность воспаления, у 2/16 (12,5%) (УПА 15 мг) — умеренная активность.</p></sec><sec><title>ВЫВОДЫ</title><p>ВЫВОДЫ: в реальной практике УПА позволяет достичь раннего клинического ответа, клинической и эндоскопической ремиссии уже после индукции при хорошей переносимости и безопасности у пациентов ЯК с неэффективностью кортикостероидов и ГИБП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>AIM</title><p>AIM: to evaluate the efficacy and safety of Upadacitinib (UPA) — an oral selective janus-kinase 1 (JAK1) inhibitor in real clinical practice for the treatment of ulcerative colitis.</p></sec><sec><title>PATIENTS AND METHODS</title><p>PATIENTS AND METHODS: in 2021-2023, as part of a multicenter, prospective, open, uncontrolled study, fortysix patients with mild-to-moderate ulcerative colitis (UC) were included (male: female ratio = 16:30, mean age 34 ± 3.1 years). Extraintestimal manifestation (EIM) initially was diagnosed in 7 patients (jonts and skin). Indications for UPA administration were: resistance to corticosteroids, ineffectiveness of basic and previously biologics treatment. The effectiveness was evaluated using the Mayo score, the Schroeder endoscopic index, as well as dynamic of hemoglobin, CRP and ESR. UPA was prescribed in accordance with the instructions for use: 45 mg for induction and 15/30 mg for maintenance therapy. The evaluation criteria were: the frequency of early primary clinical response within 1 week, the rate of clinical response/remission and endoscopic response/remission, the changes of EIM and the rate of adverse events (AE) in 8 weeks of induction and in 48 weeks of maintenance treatment.</p></sec><sec><title>RESULTS</title><p>RESULTS: all patients completed the induction course of UPA for 8 weeks. Early clinical response within 1 week with reduction of stool movements to 3 or less per day, absence of blood in stool was achieved in 25 (54.3%) patients. Clinical remission developed in 34 (73.9%) and clinical response in 3 (6.5%) of patients at the end of induction. Normal CRP and ESR levels and endoscopic remission was detected in 24 (52.2%) of patients in 8 weeks. The severity of EIM at the end of induction decreased in 4/7 (57.1%). One case of herpes zoster was registered as serious AE with drug withdrawal within the induction. The results of UPA maintenance therapy at week 48 were assessed in 16 patients. Clinical remission was maintained in 14/16 (87.5%) patients, with 11/16 (68.7%) of them receiving UPA at a maintenance dose of 30 mg per day, and 3/16 (31.3%) patients receiving 15 mg per day. In 2/16 cases (12.5%), clinical activity was maintained. Endoscopic remission developed in 10/16 (62.5%) patients, which was expressed in a decrease in the endoscopic Schroeder index decreases to ≤ 1 points, with 8 patients receiving a maintenance dose of 30 mg per day, and 2 patients receiving 15 mg per day.
