В связи с высокой вероятностью нарушения целостности кишечной стенки актуальным является выявление факторов риска глубокого повреждения вплоть до перфорации при выполнении ESD по поводу раннего рака ободочной кишки.ПАЦИЕНТЫ И МЕТОДЫ: в исследование включен 61 больной ранним раком ободочной кишки, перенесших удаление новообразований с использованием технологии ESD в период 2018–2023 гг. Глубоким повреждением считали возникновение глубокого дефекта мышечного слоя до серозной оболочки кишечной стенки с сохранением целостности последней и отсутствием прямого контакта со свободной брюшной полостью. Были проанализированы клинические факторы риска глубокого повреждения вплоть до перфорации, включая возраст, пол, морфологию, размер, расположение опухоли, продолжительность операции.РЕЗУЛЬТАТЫ: средний размер образований при выполнении ESD составил 20,0 (1,50–2,80) мм. Частота удаления единым блоком составила 81,7%. Глубокое повреждение произошло у 6/61 пациентов (9,9%). Все их удалось ликвидировать эндоскопическим способом при помощи клипирования. Неотложного хирургического лечения у данной категории пациентов не потребовалось. Унивариантный анализ показал, что размер опухоли ≥ 2,0 см (р = 0,04), локализация в правых отделах (р = 0,04), тип 2B-high по классификации JNET (р = 0,0004) и лифтинг 3–4 по Kato H. (р = 0,04) являются статистически значимыми факторами риска глубокого повреждения вплоть до перфорации стенки кишки во время диссекции.ЗАКЛЮЧЕНИЕ: размер опухоли ≥ 2,0 см, локализация в правых отделах, тип 2B-high по классификации JNET и лифтинг 3–4 по Kato H., являются факторами, ассоциированными с риском интраоперационного глубокого повреждения стенки кишки вплоть до перфорации.</p></abstract><trans-abstract xml:lang="en"><p>AIM: to identify risk factors for perforation during colorectal ESD for early colon cancer.PATIENTS AND METHODS: the study included 61 patients with early colon cancer who underwent ESD in 2018–2023. Perforation was defined as a deep muscular layer defect down to serosa with its preservation without connection with free peritoneal cavity. Clinical risk factors for perforation during ESD, including age, gender, tumor morphology, tumor size, tumor location, procedure time, were analyzed.RESULTS: the mean ESD specimen size was 20.0 (1.50–2.80) mm. The overall en bloc resection rate was 81.7%. Perforations occurred during ESD in 6 of 61 patients (9.9%). All perforations were successfully treated with endoscopic closure using hemoclips and nonsurgical management. No emergency surgery occurred.
