В 112/130 (86,2%) наблюдений выполнена илеоцекальная резекция кишечника с формированием анастомоза. В 18/130 (13,2%) случаях – резекция терминального отдела подвздошной кишки с резекцией правых отделов ободочной кишки с формированием илео-трансверзо анастомоза. Аппаратный анастомоз по типу «бок в бок» был сформирован у 57/130 (43,8%) больных, тогда как ручной анастомоз по типу «конец в конец» - у 73/130 (56,2%). Осложнения в послеоперационном периоде возникли в 21/130 (16,2%) наблюдении. После оперативного лечения в качестве противорецидивной терапии большинству пациентов был назначен азатиоприн - 112/130 (86,2%) пациентам, при этом в 31/112 (23,8%) наблюдении дополнительно назначена биологическая терапия. У 14/130 (10,7%) больных противорецидивная терапия проводилась в монорежиме биологическим препаратом.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ: прослежены с медианой в 28,5 (1,9-95,4) месяцев все 130 пациентов. Рецидив возник у 54/130 (41,5%) больных в среднем через 18±5 (12-41) месяцев после операции. Ремиссия заболевания достоверно чаще встречалась у пациентов, которым формировался анастомоз ручным способом (р=0,02). Так продолжительность операции более 200 минут достоверно ассоциировалась с увеличением частоты рецидива заболевания (р=0,03). Для выявления предикторов рецидива был проведен однофакторный и многофакторный анализ. В результате данного анализа было выявлено, что тип формирования анастомоза не влияет на риск рецидива болезни. Более того, среди значимых факторов находится длительность операции. Продолжительность вмешательства более 155 минут в однофакторной модели повышает шанс развития рецидива в 2,9 раз (р&lt;0,05), а в многофакторной - в 6,3 раза.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ: таким образом, установлено, что тип формирования анастомоза не влияет на риск возникновения возврата заболевания. Длительность операции более 155 минут повышает шанс рецидива в 6 раз (р&lt;0,01).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>AIM</title><p>AIM: to evaluate the effect of intestinal anastomosis type on risk of Crohn’s disease (CD) recurrence.</p></sec><sec><title>PATIENTS AND METHODS</title><p>PATIENTS AND METHODS: the retrospective cohort study included 130 patients with CD who underwent surgery for a complicated CD in 2012–2017. Ileocecal resection with anastomosis was performed in 112/130 (86.2%) patients. Resection of the terminal ileum with resection of the right side of the colon with the formation of an ileo-transverse anastomosis. In 18/130 (13.2%) cases. Stapled “side-to-side” anastomosis was formed in 57/130 (43.8%) patients, while hand sewn “end-to-end” — in 73/130 (56.2%) patients. Post-op complications occurred in 21/130 (16.2%) cases. After surgery, most patients were treated by azathioprine as an anti-recurrence therapy — 112/130 (86.2%) patients, while in 31/112 (23.8%) cases, additional biological therapy was done. In 14/130 (10.7%) patients, anti-recurrence therapy was carried out in mono mode with a biological drug.</p></sec><sec><title>RESULTS</title><p>RESULTS: mean follow-up was 28.5 (1.9–95.4) months. Recurrence occurred in 54/130 (41.5%) patients on average 18 ± 5 (12–41) months after surgery. Thus, the operative time exceeding 200 minutes was significantly associated with an increase in the recurrence rate (p = 0.03). It was found that the type of anastomosis does not affect the recurrence risk. Moreover, among the significant factors was the operative time. It increases the chance of recurrence by 2.9 times in the univariate model (p &lt; 0.05), and in the multivariate model — by 6.3 times, when exceeding 155 minutes.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: the type of anastomosis does not affect the recurrence risk.
