В основную группу включены больные (n = 25), которым после анкетирования выполнялась БОС-терапия и тибиальная нейромодуляция. Функциональные показатели запирательного аппарата до и после лечения оцени- вали методами аноректальной манометрии. В контрольную группу были включены пациенты (n = 25) по методу «псевдорандомизации» — подбор контрольной группы по индексу соответствия (Propensity score matching).</p></sec><sec><title>Результаты</title><p>Результаты: средний балл по шкале СНПР, у пациентов основной группы составил 41,0 ± 2,8, в контрольной группе — 38,0 ± 4,0. При сфинктерометрии средние показатели давления в покое до лечения составили 30,0 ± 7,8, при волевом сокращении 140,6 ± 56,0 мм рт. ст. После проведения курса консервативного лечения у пациентов основной группы субъективные показатели улучшились: средний балл по шкале СНПР снизился с 41,0 ± 2,8 до 17,0 ± 8,0 баллов (p &lt; 0,0001), а показатели среднего давления после лечения воз- росли с 30,0 ± 7,8 до 36,0 ± 8,0 (p = 0,004), при волевом сокращении с 140,6 ± 56,0 до 157,5 ± 53,2 мм рт. ст. (p = 0,008). Сравнивая результаты анкетирования пациентов в основной группе, т.е. пациентов, про- шедших консервативную реабилитацию, с результатами анкетирования пациентов контрольной группы после закрытия стомы и спустя 12 месяцев, оказалось, что в основной группе отмечено существенное уменьшение степени выраженности СНПР: 17,0 ± 8,0 баллов против 35,0 ± 4,5 (p = 0,0003), что свидетельствует о динамическом улучшении тонуса и сократительной способности сфинктерного аппарата на фоне проведения консервативной реабилитации.</p></sec><sec><title>Выводы</title><p>Выводы: использование комплексного консервативного подхода, включающего БОС-терапию и тибиальную нейромодуляцию у пациентов с выраженным СНПР, статистически значимо улучшает функциональные показатели запирательного аппарата прямой кишки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim: to develop a conservative rehabilitation program for patients with severe symptoms of LARS.</p></sec><sec><title>Patients and methods</title><p>Patients and methods: since January 2019, 50 patients after low anterior resection were included in the study. The main group included 25 patients who underwent biofeedback therapy and tibial neuromodulation in 3–6 months after surgery. Functional results before and after treatment were evaluated by anorectal manometry. The control group included 25 patients, according to the Propensity score matching.</p></sec><sec><title>Results</title><p>Results: the median score on the LARS scale, in the main group was 41.0 ± 2.8 points, in the control — 38 ± 4. With sphincterometry, the median pressure at rest before treatment was 30.0 ± 7.8, with a voluntary contraction of 140.6 ± 56.0 mm Hg. After the conservative treatment, patients in the main group had significantly better results: the median score on the LARS scale decreased from 41 ± 2.8 to 17 ± 8 points (p &lt; 0.0001), the median pressure after treatment increased from 30.0 ± 7.8 to 36.0 ± 8.0 (p = 0.004), with a voluntary contraction from 140.6 ± 56.0 to 157.5 ± 53.2 mmHg (p = 0.008).
