Представлено клиническое наблюдение дебюта сверхтяжелой (фульминантной) формы язвенного колита у молодого, ранее практически здорового пациента. Диагноз установлен при поступлении в инфекционное отделений районной больницы, начато этиопатогенетическое лечение. Неэффективно. Переведен в специализированное отделение областной клинической больницы в тяжелом состоянии с метаболическими и волемическими нарушениями, анемией, тромбоцитопенией, синдромом системной воспалительной реакции. Несмотря на интенсивную терапию, состояние ухудшалось, рецидивировало массивное кишечное кровотечение. По неот-ложным показаниям выполнена колпроктэктомия. Илеостома по Бруку. При патоморфологическом исследовании диагноз язвенного колита подтвержден – воспаление высокой степени активности с вовлечением подслизистого и мышечного слоев кишки с множественными крипт-абсцессами. На 26-е сутки выписан в удовлетворительном состоянии на амбулаторное лечение. Осмотрен через 3,5 года. Самочувствие удовлетворительное. Режим питания и характер пищи без ограничений. Работает по специальности.

The case report of the onset of acute severe ulcerative colitis (ASUC) in a young, previously healthy patient. The UC was revealed department of the infectious disease in general hospital, a proper conservative treatment was started but was ineffective. The patient was taken to the colorectal surgery unit of the tertiary referral center. His status was very poor including metabolic changes and dehydration, anemia, thrombocytopenia, systemic inflammatory response syndrome. Despite intensive therapy, the condition impaired and massive intestinal bleeding recurred. Urgent colproctectomy with end ileostomy was performed. Pathomorphological study verified the diagnosis of ulcerative colitis – highly active inflammation involving the submucosal and muscular layers of the intestine with multiple crypt abscesses. The patient was discharged 2 days after surgery. Follow-up was 3.5 years, patient in good condition without restrictions.
