Проблема лечения пилонидальной болезни является чрезвычайно актуальной в наши дни. От данной патологии страдает до 5% взрослого трудоспособного населения. Среди колопроктологических заболеваний пилонидальная болезнь занимает четвертое место, уступая лишь геморрою, парапроктиту и анальной трещине. Первое упоминание пилонидальной болезни появилось в научной литературе около двух веков назад. За этот период были сформированы разнообразные взгляды на этиологию и патогенез данного заболевания. Западные ученые являются сторонниками приобретенного генеза заболевания. Им противопоставлены страны бывшего СССР, где пилонидальную болезнь рассматривают с позиции врожденной патологии. Исходя из теорий этиопатогенеза были предложены различные методы хирургического лечения. При рассмотрении пилонидальной болезни с точки зрения врожденной аномалии, применяются методы, направленные на ее устранение (иссечение пилонидальной кисты с пластикой послеоперационной раны и без нее). Если же рассматривать данную нозологическую единицу как приобретенное заболевание, то ее хирургическое лечение будет заключаться в выполнении операций, разрывающих патогенетический механизм. К ним относятся: BascomI, CleftLift, EPSiT и многие другие. Каждый клинический случай требует индивидуального подхода для получения положительных результатов Любой из методов хирургического лечения пилонидальной болезни имеет определенные показания и противопоказания к оперативному вмешательству. Цель нашей работы состоит в сравнении основных методик хирургического лечения рассматриваемой нозологической единицы и определении их сильных: и слабых сторон. К сожалению, на сегодняшний день отсутствует «золотой стандарт» в лечении пилонидальной болезни.

The treatment of pilonidal disease (PD) is extremely important now. Its incidence is up to 5% of the adults. PDtakes the fourth place among such coloproctological diseases as hemorrhoids, abscess and fistula-in-ano and anal fissure. The first experience of PD was described two centuries ago. During this period, various theories on etiology and pathogenesis of the disease were suggested. The western papers support the idea that the acquired genesis is a cause of PD. This point of view is the opposite to the countries of the former USSR, where pilonidal disease is considered as congenital pathology. Numerous procedures have been proposed because of the different theories of the etiopathogenesis if you take the point of view that the PD is a congenital disease it should be mentioned that the main goal of these methodsiselimination of pilonidal cyst with closure of the wound or without. If to consider PD as an acquired disease, the surgical break of pathogenetic mechanism is a key. Thisapproach includes: Bascom I, Cleft Lift, EPSiT procedures. Every procedure for PD has certain indications and contraindications. The aim of the review is to compare the main methods and determine their strengths and weaknesses. Unfortunately, today there is no "gold standard" in the treatment of pilonidal disease.
